Anyone can make a referral by calling (808) 924.9255 and asking to speak to our Admissions Coordinator. 

Basic information will be gathered, and a Hospice Hawaii team member will call to schedule a meeting. 

There is no cost or obligation for the introductory visit.  Hospice care can begin as early as the day of the initial meeting.
