Care Team

Hospice Hawaii will care for you and your family through a team approach. You, along with your family, will be able to help plan a treatment program to fit your needs. Your interdisciplinary care team may include the following:

  • Your Own Physician - directs the care you receive from Hospice Hawaii.
  • Hospice Hawaii Physician - coordinates your care with your own physician and oversees treatment by the hospice team
  • Registered Nurse Case Manager - specially trained and certified in hospice and palliative care that manages your physical care and coordinates other services
  • Licensed Social Worker - offers emotional support, counseling, assistance with advanced planning
  • Certified Nurses Aide - provides assistance with bathing, dressing and personal grooming.
  • Chaplain - assists in identifying spiritual concerns, and offers counseling
  • Bereavement counselor - supports the patient and family, and continues with family members
  • On-call Nurse - provides support after hours and on weekends as part of the hospice care.
  • Volunteers - provides limited personal care, including respite care for family members and other caregivers, running errands, light housekeeping, preparing meals, and providing emotional support, comfort and companionship to a patient and family.

Hospice Hawaii can provide hospice care in any care setting of the patient’s choice. Based on patient preferences or needs, care can be provided in a full range of settings.

Hospice Hawaii will come to you wherever you call home. The goal of hospice care is to promote the highest quality of life possible. It has been proven that the best option for a patient’s physical and mental well being is when surrounded by family and loved ones at home.

When care at home becomes difficult, Hospice Hawaii works with the patient and family to find a care setting to best meet the patient’s needs.

Kailua Home

Located in Enchanted Lake, our tranquil home setting and ohana style care creates an inviting atmosphere. It’s a home away from home, the comfortable, short-term residence features around-the-clock nursing care, state-of-the-art pain management, emotional and spiritual support and the personalized attention of trained volunteers.
