Hospice Hawaii’s Commitment
Since 1979, Hospice Hawaii has provided care and support to terminally ill patients and their families. The comprehensive, interdisciplinary program is designed to support the choices of patients, as well as preserve their dignity and allow them the opportunity to live the remainder of their life as fully as possible in the comfort of their home or chosen setting.
Our patient care team includes medical directors, nurse case managers, certified nurses aides, medical social workers, chaplains and volunteers who provide high-quality, personalized hospice and palliative care.
Hospice Hawaii Works With You
As the primary care physician, you continue to be the patient’s doctor and to oversee patient care. Hospice Hawaii’s services allow you to continue your relationship with your patients, including billing for office or home visits. Although Hospice Hawaii’s team will be are assisting with the care, these will still remain your patients. We expect you to continue the relationship in the way you want to have it.
Hospice Hawaii’s Referral Process
You can refer a patient opting to receive Hospice Hawaii services at anytime. Contact our Admissions Coordinator at 808-924-9255. A member of the Hospice Hawaii team will contact the patient to schedule and meeting. Hospice care services can begin as early as the day of the initial meeting.
Our Services Include:
- Nursing care focusing on comfort and relief from pain and other distressing symptoms
- 24-hour on-call availability
- Assistance with a patient’s personal hygiene
- Emotional and spiritual support for patients and their families
- Assistance ordering medical supplies and equipment
- Instruction and support to help family members care for their loved one
- Assistance with respite care, companionship, meal preparation, errands, and direct patient care by specially trained hospice volunteers
- Spiritual counseling for all faiths
- Bereavement counseling and support for family members
- Assistance with benefit applications and documents such as living wills or advanced directives
- Alternative Therapy: Including Aromatherapy, Art Therapy, Massage Therapy and Pet Therapy
