As a flexible and dynamic health care provider in the State of Hawaii, Hospice Hawaii is always looking to add the best professionals to our team. Hospice Hawaii provides both home-based care, involving traveling to the patient’s and their family’s homes. We also operate two hospice residential units, each with 5-beds, staffed 24 hours per day with RNs or LPNs and CNAs.

We are currently accepting applications for the following positions:

Patient Care Coordinator (PCC)

The PCC leads a multi-disciplined care team based upon the shared values, mission and vision of Hospice Hawaii. Coordinates team activities to achieve the highest quality patient & family care outcomes.

The ideal candidate must have strong organizational and time management skills as well as the ability to critically analyze and problem solve issues. Excellent, effective and professional verbal and written communication skills are essential to this position. The PCC must be able to demonstrate proficiency in MS Office applications and the ability to become rapidly proficient in our Electronic Medical Records system. Must be an RN (BSN) with current State of Hawaii licensure (in good standing) and have at least 3 years experience in hospice or related healthcare field. Position requires a valid driver’s license and frequent use of own car with valid registration and insurance. Full time, salaried position with benefits.
