Registered Nurse (Float) – Case Management/Admissions/After Hours (FT)
Will perform Case Management, Admissions and possibly After Hours duties using the principles of palliative care and holistic philosophy to provide care that reflect skilled nursing judgement, medical necessity, measurable patient caregiver objectives and reasonable goals. The role will include 2-3 days per week working as a Nurse Case Manager and the other days, as an Admissions Nurse. As a Nurse Case Manager, primary duties include but are not limited to provide case management services to best benefit patients and caregivers; care provided reflects compliance with federal and state regulations; documents all nursing and case management activities in accordance with hospice protocol; participates in Interdisciplinary Team conferences.
As an Admissions Nurse, they will assist with determining eligibility for hospice care and facilitates the coordination of response and care of our hospice team. They assure completeness of all intakes and admissions consistent with Hospice Hawaii policies and procedures and Medicare Conditions of Participation; assures documentation of all patient care activities by the admissions team in accordance with hospice protocols and develops and maintains relationships with nursing facilities and their staff to provide optimal hospice service to resident in nursing facilities.
May be required to assume after hours, holidays and/or weekend work as needed.
Must have current Hawaii license as a Registered Nurse; BSN preferred or equivalent to a Nursing degree; at least one year of experience in a healthcare or gerontology.
RN/LPN/CNA (Hospice Aides) – Community and Kailua Home (per diem)
RNs needed for case management/admissions/after-hours coverage, LPNs for shift coverage at our Kailua Home, and Hospice Aides to care for patient in both settings.
The ideal candidates will possess or demonstrate the ability to provide care and comfort to our patients and their families and will collaborate with the care team on an ongoing basis to help assure quality and continuity of care.
RNs and LPNs must have current State of Hawaii RN or LPN license. Hospice Aides must have current State of Hawaii certification as a home health aide or nurse’s assistant. One year of direct patient care experience preferred.
Community: Generally Mondays through Fridays from 8 am – 4:30 pm
After Hours shifts: 4 pm – 12 am, 12 am – 8 am and on Sat/Sun: day shift 8 am – 4 pm
Our Kailua Home is a 24/7 inpatient facility. Shifts are 12 hours each:
DAYS: 6 am through 6:30 pm
NIGHTS: 6 pm through 6:30 am.
Requires after hours’ work including weekends and holidays
Social Worker, Pediatric and Adult (FT)
Responsibilities include, providing crisis intervention, individual, couple and group therapy to patients and families for a diverse range of issues related to grief/loss, adjustment to illness, relationships/family dynamics, mental health, abuse, symptom/pain management, spirituality and bereavement. Participates in the provision of social work services in a transdisciplinary care team setting and collaborates with team members in developing a patient centered plan of care.
Must have a Master’s Degree in Social Work from an accredited school of social work and a current license to practice as a Licensed Social Worker (LSW) in the State of Hawaii. At least one year of experience in a related social work field.
Human Resources Coordinator (FT)
Responsible for recruiting, onboarding/offboarding employees, processing payroll and benefit changes and advising employees on policies/procedures, state and federal labor laws. Will oversee employee recognition and engagement, benefits open enrollment and other events.
Must have at least two (2) years’ experience in both Human Resources operations as well as payroll administration. Solid knowledge of state and federal laws to include but not limited to FMLA, HFLL, ADA, EEOC, OSHA. Proficient in Microsoft Office applications. (Word, Excel, PowerPoint). Requires excellent verbal and written communication skills. Flexible, organized and detail oriented with the ability to professionally communicate staff, vendors and patients/families (as needed).
