Community Liaison (FT)

Responsible for generating and responding to new referrals from hospitals, nursing facilities and other community partners . These skilled sales professionals who possess the demonstrated ability to develop and cultivate strong relationships with our referral sources through outreach, education and networking. Must have a Bachelor’s Degree or equivalent in Marketing, Communications, Healthcare Administration or other related fields and a minimum of one year direct sales experience; preferably in the healthcare environment. Having knowledge of hospital/physician referral process and end-of-life care is preferred.

CNAs (Per Diem)

Experienced CNAs needed to care for our patients at our Kailua Home. Qualified candidates must be available for 12-hour shifts (Day – 7:00 am through 7:00 pm or Night – 7:00 pm through 7:00 am). Must have current State of Hawaii certification as a home health aide as well as one year of direct patient care experience.

Director of Clinical Operations (FT)

Responsible for the overall coordination and delivery of patient services delegating responsibility as appropriate to program managers and coordinators. Oversees delivery of patient services and compliance with agency policy in all patient care settings, including care provided in the patients’ homes, in various hospitals and nursing homes, neighbor island program(s) and in Hospice Hawaii’s Homes. Current Hawai‘i license as a Registered Nurse. Bachelor of Science in Nursing required, Master’s in related field preferred. At least 3 years management experience in a health care setting required. Desired three years supervisory experience in a home health agency, hospice, or related program.

Electronic Medical Records (EMR) Coordinator (FT)

Supports and trains our clinicians and other staff on the efficient use of our EMR software program. The EMR Coordinator collaborates with the EMR vendor and our clinicians to identify, resolve and/or escalate practice concerns. Must have Bachelors in Education, IT or related field and minimum 1-3 years healthcare experience; must be proficient in Microsoft Excel, Outlook and Access

Nurse Case Managers (FT)

Provides case management services to best benefit our community patients and caregivers needs in a manner that reflects compliance with federal and state regulations; documents all nursing and case management activities in accordance with hospice protocol; participates in Interdisciplinary Team conferences and may be required to assume on-call duties after hours on holidays or weekends as needed. Must have a current Hawaii RN license. BSN preferred; and at least one year of experience in a healthcare or gerontology setting.
