Admissions Nurse (FT)

Skilled, confident RNs needed to meet with, assess and admit patients/families to our service. Provides support/education to community health care providers on the benefits of hospice care. Must have active HI RN license and two years of experience in acute care setting, hospice/home care and sales experience preferred.

Chaplain (FT)

Chaplain needed to provide spiritual care in a multidisciplinary team approach. Provides compassionate spiritual and emotional support to patients, family members and staff, regardless of faith background or religious affiliation. Bachelor’s and Master’s degrees in Divinity or Theology required.  At least one unit of Clinical Pastoral Education and two years of ministry experience preferred. Ability to use an electronic medical record system.

Community Liaison (FT)

Community Liaison sought to develop and maintain strong, positive relationships with physician’s offices, hospitals, nursing facilities, and other community partners to facilitate referrals of patients for hospice care. The liaison provides education to patients and their caregivers about hospice, evaluates for eligibility and works closely with the physician and interdisciplinary team to develop an initial plan of care using the principles of hospice and palliative care.

Medical Director (FT)

Compassionate physician who will be responsible for the medical component of the agency’s patient care program. Complements the attending physician’s care and acts as a medical resource to the Clinical Team members and assures continuity of Hospice Hawaii’s medical services. Serves as liaison with community physicians, medical schools, and teaching hospitals. Must possess a valid medical license to practice in the State of Hawaii and DEA certificate. Certification in Hospice and Palliative Medicine and certification as a Hospice Medical Director desired.

Volunteer Coordinator (FT)

Volunteer Coordinator needed to recruit, select, train, schedule, and support volunteers. The Volunteer Coordinator demonstrates commitment to the philosophy and goals of hospice care and works closely with the Interdisciplinary Care Team, in ensuring volunteers are appropriately matched to assist patients and families. Bachelor’s Degree preferred with minimum two years of experience in managing and supervising individuals and skilled in interpersonal relations, organizational development, and program management.

CNAs (FT and per diem)

Experienced CNAs needed to provide patient care to our patients in the community OR to cover shifts at our 5-bed Kailua Home.

In the community, will drive to patient’s homes and other facilities on Oahu. Hours of work are generally Monday through Friday from 8 am – 430 pm.

At Kailua Home, must be available for 12-hour shifts (Day – 7 am through 7 pm or Night – 7 pm through 7 am).

Must have current State of Hawaii certification as a home health aide as well as one year of direct patient care experience.

RN Case Manager (FT and per diem)

RNs with hospice or home care experience needed to provide care to our patients across Oahu. Experience in med/surg or oncology helpful. Must have active HI RN license.

LPNs (per diem)

We need LPNs with hospice or home care experience to care for our patients and families in our Kailua Home. Experience in med/surg or oncology helpful. Must have active HI LPN license.

Social Worker (per diem)

Experienced Social Worker needed to provide support to our patients and families across Oahu. Must have MSW and LSW.
