Hospice Hawaii’s unique, interdisciplinary approach to care provides for the physical, social, emotional, and spiritual well-being of all those we serve. Because each person is different, we assemble a care team based on the patient’s individual needs and choices. Specialists may include:
- Personal Physician
Directs the care you receive from Hospice Hawaii - Hospice Medical Director or Nurse Practitioner
Develops the patient’s plan of care with the patient’s own physician and oversees treatment by the hospice team - Registered Nurse Case Manager
Coordinates patient’s overall care - Social Worker
Offers emotional support, counseling, resources, and assistance with advance planning - Hospice Aide
Provides assistance with bathing, dressing and personal grooming - Spiritual Care
Chaplains assist in identifying spiritual concerns and offers counseling - Volunteers
Provides limited personal care, including respite care for family members and other caregivers, running errands, light housekeeping, preparing meals, and providing emotional support, comfort and companionship to a patient and their family. Volunteers also provide calling services to patients and families. - Specialized Therapies
Offers patients and their families a variety of alternative therapies including massage therapy, art therapy, aromatherapy, music therapy, healing touch and pet therapy - Bereavement Support
Supports the patient and family, and continues with family members after patient passes - After Hours Nurse
Provides support after hours and on weekends - Counselor
Offers other benefits such as dietary counseling, physical therapy or speech therapy, if needed
